  • Jersey City, New Jersey, Job Summary: The Nurse Practitioner functions as an independent advanced practice professional in an expanded role. This requires collaboration with the interdisciplinary team to provide for assessment and therapeutic management of patient/family needs. Care is provided to hematology oncology patients in the ambulatory and hospital settings. The NP will work alongside the physicians and the clinical team within the multidisciplinary oncology service line, designed to meet the needs of cancer populations within the continuum of care; cancer screening, diagnosis, treatment, survivorship, etc. The major responsibilities include expanded patient assessment, triage, diagnosis, coordination and documentation of care. Management to restore, maintain and improve the health status of patients. In addition, the NP is responsible for reporting identified safety issues such as hazardous and medical errors.       Essential Functions: Performs comprehensive initial and episodic health histories and physical exams; in order to identify existing or potential health problems. Initiates and implements an individualized nursing and medical plan of care in collaboration with physicians and members of interdisciplinary team. Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician. Provides follow-up care including acute and routine examinations. Assesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physician. Monitors and adjusts patients’ medication according to protocol or medical plan of care including adequate pain control. Implements nutritional therapies, as appropriate. Triages patient problem calls in office, which requires medical attention. Orders appropriate laboratory and diagnostic procedures. Recognizes normal and abnormal laboratory findings and the pathology involved. Documents information in patients’ charts following documentation standards. Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching). Manages medical emergencies in office, as indicated, and according to standing orders. Provides monitoring and continuity of care between health care settings. Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues. Initiates proper referrals to private or community agencies. Acts as a resource and teacher for other members of the health care team. Shares knowledge and expertise with staff. Collaborates with Surgeons, radiation oncology, inpatient nursing and medical staff as a partner to coordinate continuity of care. Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated. Supports the research/clinical trials mission by collaborating with the research team. Identifies and screens patients for appropriate eligibility for protocol study participation and aids in the accumulation and recording of date regarding patients on protocols. Reviews and audits charts routinely. Performs other related duties as assigned.     Job Requirements: Registered Professional Nurse with current NJ state license. Master’s degree in Nursing required. NP Certification/License; CDS and DEA required. BLS  and ACLS certification required. ONS / ONCC Chemotherapy Immunotherapy Certificate highly preferred Oncology experience highly preferred.   • Dover, Delaware, Bayhealth is Central and Southern Delaware’s largest health care system, Bayhealth is comprised of Bayhealth Kent Campus, Bayhealth Sussex Campus, the freestanding Emergency Department in Smyrna as well as numerous satellite facilities and employed physician practices encompassing a variety of specialties.   General Summary: The Cancer Institute Manager is accountable for the system-wide clinical and operational functions of the Bayhealth Cancer Institute staff on both hospital campuses and in the community that includes cancer patient navigation, survivorship, clinical trials, genetic counseling, and other cancer supportive care programs.  Also responsible for all grants, community outreach programs including cancer screening and prevention services, and for availability of funding for those in need.  Collaborates with other members of the oncology leadership team to assure timely, effective and appropriate care is delivered within productivity and budgetary standards.   Essential Functions & Accountabilities: Cancer Patient Navigation Program: Oversees the performance of Cancer Nurse Navigators and administrative functions of the Cancer Patient Navigation Program. Ensures accreditation standards are met with survivorship, and timely navigation of cancer patients.  Works collaboratively with the Medical Director of the Bayhealth Cancer Institute assuring effective and timely resolution of all issues.  Collaborates with Service Line leadership on the navigation of cancer patients. Clinical Research Program: Oversees the performance of the Clinical Coordinator – Clinical Trials and associated staff in the Bayhealth Cancer Institute’s Clinical Research Program. Ensures all accreditation and national cooperative group enrollment targets at met.  Ensures compliance with regulatory standards and audit controls. Genetic Counseling Program: Oversees the performance of the Bayhealth Cancer Institute’s Telegenetics Program. Ensures and monitors process for the timely delivery of genetic counseling results. Cancer Supportive Care Program: Oversees the development, implementation, and monitoring of cancer supportive care programs (i.e. psychosocial assessments, support groups). Ensures and monitors patient referrals to appropriate programs. Community Outreach and Education Program: Assures system-wide cancer screenings are appropriate by type of screening, audience and geography/community. Manages all cancer outreach services assuring selection of the most appropriate venues and assigning appropriate resources (i.e., staffing) for scheduled events. Ensures all accreditation standards are met with Bayhealth’s community outreach plan each calendar year. Programmatic Development: Collaborates with the Senior Director of Operations, Oncology Service Line on future planning and offerings set by the Oncology Service Line physician leadership. Collaborates with Service Line leadership on the implementation of new programs. Grants and Contracts: Monitors staff activities assuring grant-funded staff follow all funding guidelines. Coordinates all grant reports with the Bayhealth Foundation assuring timely and complete submission of all reports, invoices, etc.; monitors financial status at least quarterly and requests grant changes as needed All other duties as assigned within scope and range of job responsibilities.   Minimum Education and Experience:   Minimum Preferred Education: Registered Nurse with BSN; or Bachelor’s Degree in Health or Business Administration   MSN; or Master’s degree in Health or Business Administration or related field. Experience: Five years management/supervisory experience, to include experience with leading multidisciplinary teams.   Prior experience with program management and physician relationships.  Prior experience in a matrixed environment and program development.    Six+ years experience in related healthcare role.   Outpatient oncology managerial experience or other clinical area management experience.   Prior experience with cancer program management and/or physician practice management. Certification/ Registration: Current licensure in the State of Delaware, or compact state  Oncology nursing certification (OCN), FACHE Computer/Software: Proficiency in Word, Excel, and PowerPoint Presentations.   Experience with EPIC and skills in data analysis.   Special Knowledge, Skills, and/or Abilities: Excellent organizational and time management skills, ability to meet multiple deadlines, effective communication skills, ability to function under conditions of high pressure. Must be able to extract and analyze data from a variety of information systems. Must be able to cultivate appropriate business relationships with physicians, service line staff and other Bayhealth individuals and groups Must be proficient in reading, writing, and speaking English. Preferred: Experience in project management, education, case management, or utilization management.
