Texas Cancer Survivorship Programs-- Assembling Master List

Hello! UT Southwestern Medical Center in Dallas' Cancer Genetics program is the recipient of a grant through the Cancer Prevention Research Institute of Texas (CPRIT) entitled, The Dissemination of a Genetic Navigation Framework for Hereditary Cancers to Increase Mutation Carrier Identification and Improve Outcomes.

As part of this grant, we are creating an online genetic patient navigation toolkit.  I am reaching out today to gather information as to what cancer survivorship programs are available within the state of Texas, as well as contact information for each one.  The purpose is two-fold:
1.) I will list these in one of our toolkit documents, Texas-Based Cancer Survivorship Programs.  These toolkit documents will be available (free of charge) to anyone via our UT Southwestern Cancer Genetics professional portal.  It is a work in progress.  https://utswmed.org/conditions-treatments/genetics-and-hereditary-cancers/guide-hereditary-cancer-health-pros/genetic-screening-and-navigation-toolkit/
2.) I  will also use these contact names from the respective programs to help us with a brief telephone focus group as to what needs they see from a survivorship navigation standpoint for these high-risk patients.

I have tried to search via several ways.  I see the existence of survivorship programs out there, however not having the best of luck for specifics and/ or contact information.

Your help in this is extremely appreciated!  I realize all are under an unprecedented amount of stress right now.  If you could please take a moment in the next several days (by next Friday, 4/3) to share with me the contact information (name, email/ phone) of your center's survivorship program coordinator, that is all I need.  It can be sent to me at kathy.pratt@utsw.edu.

Thank you very much in advance for your help in making these contacts so we can make the Genetic Navigation toolkit as useful as possible!  Like many, I am working from home these days. Thank you for your time and consideration!  Especially during these times!

    Kathy

Our survivorship program is coordinated through the Harrington Cancer and Health Foundation.  Their number is 806-331-2400.  Pam McMillan is our contact.

    Thanks,
