UPDATED DINNER TERMS
If you sign up for a dinner lecture, it is with the understanding that you have read, understood, and agreed to the following terms.
· SIGN-UP. I have signed up for myself only.
· HEALTHCARE PROFESSIONAL. Attendance at dinner lectures is exclusive to healthcare professionals and ONS Hawaii Chapter Members. All Attendees are required to be an active ONS Hawaii Chapter Member.
· REGISTRATION. Free admission for current ONS Hawaii Chapter Members. I alone am responsible for maintaining my membership.
· CANCELLATIONS. I will cancel myself by emailing info@ONShawaii.com. The deadline for cancellations is 9am 2 days before the program. If I cancel after this time, I will negatively impact my ability to sign up for future dinner lectures and a fee of $25 will be directly invoiced to me. Registrations are non-transferable, an individual showing up to a dinner under another individuals’ registration will be turned away as available seats will be given to members on the waitlist.
· NO-SHOW. If I am a no-show to the dinner lecture and have not contacted the Board member listed for the dinner, it will affect my ability to sign up for future dinner lectures and a fee of $25 will be directly invoiced to me.
