When you have a dual implanted port, what is the proper documentation name for each side? Medial/lateral? Inside/outside? Right/left?
    Thank you!

Noting orientation of the port is important to document which side of the port (lumen) you have accessed. For laymen terms you can use the top or bottom access or left and right as though you are looking straight at it. Medical terms can be cephalad (top, anterior or head) or caudal (tail, posterior or bottom); or left and right depending upon side of body port is on could be medial (closer to midline) or lateral (closer to the side). Best always.

At my current institution we use a lot of double ports. We are currently revising our procedures for central line care. This is what the new procedure will look like for proper identification of double ports.

    I would always use medial/lateral

I have always documented and seen it documented by anatomical terms such as medial/lateral, superior/inferior, etc. That's really the safest way to document for all to understand. Any healthcare provider could look at that documentation and know which side/port you are referring to.

We also label double ports as medial/lateral and superior/inferior.

For the double port we use distal or proximal lumen. Proximal meaning closest to the center of the body
