follow-up

In preparation for beginning our oral chemo education process, I have been very busy reading articles, shadowing our IV nurse educators as well as our insurance authorization administrator. I have a loosely outlined process for the education of our new start oral chemo patients. I am pondering the gray area of patient follow-up. I see that many of you call the patient weekly for 3-4 weeks after the formal education but am unclear of further follow-up. Most of our physicians see the oral chemo patients monthly for labs and MD appt. Do you leave the follow-up to the provider or is there a process in place for the educator? Please advise.
    Thank you

    Nancy,
My policy is followup 5-7 days post start, then weekly for additional 3 weeks, then monthly for at least 5 more months.  But follow up also depends on patient need and the required monitoring parameters for the specific drug. If blood tests needed every 2 weeks, then I follow up every 2 weeks.   Every refill request from pharmacy also generates a followup.  I define follow up as checking into the patient record for documentation related to the oral oncolytic.  Has the patient been to the clinic recently, is there documentation about the oral oncolytic, adherence, toxicity assessment. Have monitoring parameters been met.   If not, then I reach out to the patient to complete these tasks.

    Good morning Elizabeth,
Per the suggestion of my administrators, I am seeking an opportunity to shadow an oral chemotherapy nurse educator. I am in the panhandle of Florida and have not been successful in my search. Are you aware of any facility that I might contact for such an endeavor?
