    ​We currently see patients that get HiDAC as an outpatient. Are there any clinics in which the RN's do the neurologic checks to assess cerebellar function (signature log, gait check etc.) prior to the cytarabine dose on day 1, 3, and 5? We have been having this done by an APP or physician and we are getting a little push back from the section chief that this can be done by the nurses.

    We give HiDAC in inpatient setting and yes, we do check neurological status which includes signature and gait check with every dose.

    We currently have the bedside RN perform a cerebellar exam (we included most of the CARTOX 10 grid) which includes orientation, speech, cognition, a signature, nystagmus, gait, and rhomberg test) before each dose of high dose cytarabine​. We have a smartphrase that the nurse uses to pull a chart into the nursing note. We hope to have flowsheet rows built within EPIC for "toxicities" that will include a cerebellar exam. As we are developing a plan to provide HiDAC in the ambulatory setting, we are still trying to decide what to do about the assessments. I believe it will be an APP before the first dose on Day1 and then the RN on subsequent days, but again, inpatient-it is the RN doing the cereblellar exam.
