Chemotherapy/biotherapy consents

    I work at a small infusion center located inside a hospital. We have never used consents for chemo/biotherapy before, however, we would like to begin doing so. Does anyone have any recommendations on what to include in the form? Any generic consent forms you'd be able to share? Thanks so much!

    Hi Mindy,

    Our practice uses a Chemo/Biotherapy/immunotherapy consent form which has the pt's full name, dx, MD, tx regimen ordered. Also includes possible and potential S/E, AR, HSR, that such were discussed with the pt; the form states that there is no guarantee for cure. Also includes pt's responsibility to inform the practice for any insurance changes, and that pt is responsible for any co-pay.

    The ASCO website has a template. It contains all the elements required for informed consent.
