Hi,
 We give high doses of Methotrexate to our inpatient lymphoma/leukemia patients. We currently have a very complicated algorithm for the nurses to use based on the current MTX level. Does anyone else have an algorithm, orders or some type of standardization tool for ordering the leucovorin based on the daily MTX level. If so, could you share with me?

We use the algorithms in Up to Date.  They are different depending upon the excretion of methotrexate (i.e. normal elimination of methotrexate, delayed late methotrexate elimination, and delayed early methotrexate elimination and/or evidence of acute renal injury).

Hi Members,

High dose MTX regime has many stages from  pre hydration, MTX administration and leucovorin administration and post hydration.

We all know that is all dependent on the Urine pH, MTX level and fluid intake and if patient is fluid overloaded.

We have several haematologist who have different requirement.

This does confuse the nurses.

I would love to read the algorithm any body has too share.
